📝 Medical Responsibility
Before entering sacred space, you must complete the Medical Disclosure Form in full and confirm that your doctor has cleared you to participate.
The Sacred Teachers are part of our Religious Belief & Sacraments, and we approach them with reverence and responsibility. This includes being honest about your physical and mental health.
It is your responsibility to:
• Submit the completed medical form prior to ceremony
• Disclose all medications and health conditions
• Ensure you are medically cleared
No one may enter the medicine space without completing this process.
